Description

Focused ultrasound in aesthetic medicine – HIFU, MFU-V, 12D HIFU and related technologies

This course is designed for doctors and advanced practitioners working with energy-based devices who want to organise and deepen their knowledge of every clinically relevant focused ultrasound technology used on the face and body. It focuses on ultrasound beam physics, selecting depth and energy, differences between treatment platforms, patient selection, anatomical safety and building combination protocols that improve lift, tissue firmness and skin quality.

You will learn how focused ultrasound works at histological and clinical level: the creation of thermal coagulation points in precisely selected tissue layers, the induction of neocollagenesis and extracellular matrix remodelling and, in selected indications, its effect on fibrous septa and subcutaneous tissue. In practice this means working in the deep dermis, the subcutaneous layer and – with selected systems and the right indications – around the superficial musculoaponeurotic system (SMAS).

Particular attention is given to the difference between HIFU and MFU-V. In the scientific literature MFU-V is described as micro-focused ultrasound with tissue visualisation, allowing more precise depth planning and a personalised treatment. Consensus statements and reviews show that MFU-V performs strongly in non-surgical lifting and tightening of mild to moderate laxity, especially of the brows, submental area, neck, lower face and décolletage.

We also discuss devices marketed as 12D HIFU and similar multi-line platforms. From an expert perspective, the focus is not on marketing names but on the parameters that really matter clinically: frequency, focal depth, cartridge type, line density, energy profile, visualisation, pulse repeatability and anatomical safety. Studies and consensus statements show that these features – not the brand name – determine treatment quality and predictable results.

This is an advanced course that teaches not only how to use the technology, but above all layer-based thinking: when the goal is lifting, when tightening, when improving skin quality and when reducing mild laxity on the body. You will learn to design treatments around anatomy, the biomechanics of ageing and the realistic potential of each platform. Systematic reviews show that focused ultrasound gives the best results in patients with mild to moderate laxity, and that outcomes depend on correct selection, planning and patient expectations.

Course programme

HIFU Expert for Aesthetic Practitioners (MHIFU, 12D HIFU) – training course, SyneViaMed

1. Scientific and technological foundations

  • physics of focused ultrasound
  • HIFU vs MFU vs MFU-V
  • thermal coagulation points: size, depth, distribution
  • the role of frequency, energy, line length and line spacing
  • effects on the dermis, fibrous septa and subcutaneous tissue
  • platform overview: systems with and without visualisation
  • manufacturer data vs clinical data
  • limitations of the technology and the most common operator errors

2. Procedural facial anatomy

  • layered anatomy of the face and neck
  • SMAS, retaining ligaments, fat compartments, danger zones
  • course of vessels and nerves in relation to safety
  • high-risk areas: temporal, mandibular, periauricular, infraorbital, anterior neck
  • treatment mapping of the face and how to avoid sensitive structures

3. Patient selection

  • indications: mild to moderate laxity, tissue descent, loss of contour and firmness
  • absolute and relative contraindications
  • matching the patient to the technology – not “selling one device to everyone”
  • when focused ultrasound is not the best choice
  • photo documentation, assessment scales and planning results over time

4. Treatment protocols – face

  • brows and forehead
  • temples and lifting the tail of the brow
  • cheeks and midface
  • lower face and jawline
  • submental area and neck
  • décolletage
  • choosing depth and cartridges for each indication
  • number of lines, energy density, pass sequence
  • lifting vs tightening treatments

5. Treatment protocols – body

  • upper arms
  • abdomen
  • above the knees and thighs
  • buttocks and areas of mild laxity
  • when treatment makes sense for skin laxity – and when it cannot replace surgery
  • matching depth and energy to tissue thickness
  • documenting body results and managing expectations (body consensus confirms MFU-V is useful especially for mild to moderate laxity of the abdomen and arms, with strong emphasis on patient selection and planning)

6. Safety, complications and management

  • procedural pain and how to control it
  • erythema, swelling, temporary tenderness, paraesthesia
  • rare neurological complications and how to avoid them
  • analysing a complication: selection, mapping, parameter or technique error
  • post-procedure standards and long-term follow-up (safety reviews show most side effects are mild and temporary, but safety depends on anatomy, training and correct technique)

7. Combining focused ultrasound with other treatments

  • principles of multi-layer therapy
  • when to combine technologies and when to space them out
  • sequencing treatments by target layer and clinical goal
  • 3-, 6- and 12-month treatment plans for the face and body

8. Expert workshop

  • case analysis
  • building protocols for different ageing phenotypes
  • “lifting first” vs “skin quality first” patients
  • choosing technologies and combination therapies
  • documentation and follow-up

Which treatments to combine for better results

The most clinically sound approach is to combine focused ultrasound with procedures that work at other tissue levels. HIFU/MFU-V mainly lifts and tightens the deeper layers, so the best combination protocols complement this with improved skin quality, biostimulation and volume restoration where needed.
